Conditions

  1. This service is for the old or sick patients of Mandiram Hospital
  2. Home delivery location shall be within 10 km radius of hospital
  3. Patient shall provide his / her Hospital number
  4. Medicines requested shall be prescribed by a doctor working in Mandiram Hospital . Requested medicines shall be cross-checked against patient’s file kept in hospital
  5. Delivery shall happen between 2:00 PM to 5:00 PM
  6. A minimum purchase for Rs 250 is required for the service
  7. Mode of Payment shall only be by cash
  8. Rs 100 will be charged per delivery
  9. Hospital reserves the right to deny this service to any person at any time.

Phone Call Request

  1. Patient or caregiver shall call to 24 x 7 number – 0481 2578393
  2. Patient or caregiver shall request telephone operator to connect to 24 x 7 Pharmacy for home delivery of medicines
  3. Patient or caregiver shall provide the address and phone number of home delivery location to pharmacist
  4. Patient or care giver shall provide the following information to pharmacist

    • Name of the Patient
    • Patient’s hospital number
    • Name of the medicine(s) and the number of tablets
    • Name of the doctor who prescribed the medicine(s)

WhatsApp / SMS Request

Patient or caregiver shall WhatsApp / SMS the following information to 9495876739 or 9961331694.

    • Name of Patient
    • Patient’s hospital number
    • Name of the medicine(s) and the number of tablets
    • Name of the doctor who prescribed the medicine(s)
    • Address of the location of delivery

Based on the request

  1. Pharmacist shall check the prescription against patient’s file
  2.  Pharmacy shall check the availability of medicines
  3.  Pharmacy shall call the patient or caregiver and inform the availability and cost of medicines
  4. Patient or caregiver shall keep the exact change for medicines + delivery charges
  5. Hospital staff shall home deliver the medicines  between 2:00 PM – 5:00 PM the next day
